Who is the controller?
Eyelid Surgery Guide operates eyelidsurgeryguide.co.uk and is the controller of the personal data you give us. Contact: hello@eyelidsurgeryguide.co.uk.
What we collect
- Enquiry form: name, email, optional phone number, nearest city, the procedure you would like to discuss and your timeframe.
- Website usage: standard server logs. If analytics are switched on, we ask for cookie consent first.
The procedure you are interested in is health information, which is special category data under Article 9 of UK GDPR. We do not ask for medical history, photographs or medication details. Please don’t send them.
Why we use it, and on what basis
We use your enquiry to pass it to the partner clinic named on the page for your city, so that clinic can contact you about a consultation. Our lawful basis is your consent (Article 6(1)(a)), and for the health information your explicit consent (Article 9(2)(a)), given by ticking the consent box. You can withdraw consent at any time by emailing us. Withdrawal doesn’t affect anything already shared.
Who receives it
Only the one partner clinic for the city you choose, named on that city’s page and on our how this works page. The clinic becomes a separate controller of your data once it receives it. If your city has no partner, we do not pass your enquiry on. We never sell your data or use it for unrelated marketing.
How long we keep it
We keep enquiry records for 12 months after we pass them on, so we can answer questions or complaints, then delete them.
Your rights
You can ask to see, correct or delete your data, object to its use or withdraw consent, by emailing hello@eyelidsurgeryguide.co.uk. If you are unhappy with how we handle it you can complain to the Information Commissioner’s Office (ico.org.uk).
Where it is stored
Enquiry data is stored and processed in the UK.
